Third-party reproduction makes it possible for people who cannot conceive or carry a pregnancy to welcome a child into their lives. As a gestational carrier/surrogate, you carry a child on behalf of the intended parent(s), offering them the gift of life.


As aย donor, you share a part of yourself โ€” egg, sperm, or embryo โ€” to make conception possible. Your decision to participate as a carrier or as a donor is an act of compassion and courage.

What this evaluation is

A focused mental health assessment for reproductive decision-making and documentation needs for surrogates and donors

A reproductive psychological evaluation is an in-depth and informative discussion that clarifies personal history, current functioning, and emotional preparedness and stability for the journey. It includes a clinical interview and PAI measurement.

This is neither a therapy session nor therapeutic relationship.

What is included

Components of a reproductive evaluation

Every evaluation is tailored, but most include the following elements. We will explain requirements, options, and accompanying documentation.

Clinical interview

A structured clinical interview covering mental health history, current functioning, support system, stressors, and relevant reproductive context.


Standardized measures

Evidence-based assessment (PAI) to support clinical impressions and strengthen documentation.


Risk and safety screening

A careful review of safety concerns and protective factors with resources provided when needed.


Written report and documentation

Clinical documentation of findings aligned with the purpose of the evaluation provided to the clinic or agency.

Submit the secure request form or email for a reproductive mental health assessment and indicate any relevant information to share. If partnered, include your partner’s name.

Sava Healing will provide the written report within the agreed timeline to the clinic or agency. If follow-up information is needed, we will contact the gestational carrier or gamete donor.
